Thank you for choosing Life In A Blender Family Medicine for your medical care!

To help familiarize you with our crazy jungle – otherwise known as our office – we have listed some of our general policies below. If you do not find what you are looking for here, please feel free to contact us. We are looking forward to meeting you!

Appointments

We see patients by appointment. A New Patient appointment may take up to an hour or more. We cannot guarantee times, but we will always try our best to accommodate you on the same day when you are suffering from an injury or illness. Please try to arrive early, at least 10 minutes before your scheduled time, so that you may complete necessary paperwork and to allow time for your records to be assembled. We make every attempt to keep on a timely schedule.

Current patients may request appointments via our Patient Portal system .

Please remember to bring along your identification card, your insurance card, and a list of any medications or supplements you are currently taking. We need to know about your current medicines or vitamins – whether over-the-counter or prescribed – as studies have found that some health-related problems can result from side effects, medications not being used as prescribed, or from interactions that can occur from combining certain medicines and supplements.

Please give our office at least a 24 hour notice in the event of appointment cancellation.

Holidays

Our office is closed: New Years Day, Good Friday, Memorial Day, July 4th, Labor Day, Thanksgiving, Christmas Eve, and Christmas Day.

Payments and Insurance

All co-payments, deductibles and non-covered services are required to be paid at the time of service. For your convenience, we do accept checks, cash, debit, and credit cards.

Uninsured Patients

We try to provide medical care for those who have no health insurance coverage whenever it is possible to do so. For such patients, we may be able to offer an initial health examination, medical history review, physical, basic lab work, or office visits at a lower cost. For more information, please Contact Us.

Prescription Refills

You can login to our Patient Portal system to request a refill of your medication.

When requesting refills to our office by phone, please be prepared to provide us with the name of the medication, the dosage information, and the phone number for your pharmacy.

Please allow us until the end of our business day to get your refill request processed and sent to your pharmacist.

As a general rule, we do not process refill requests after hours or on the weekends – unless we can do so for an emergency situation.

Life In A Blender Family Medicine Patient Portal

Information regarding your visit, office notes, and some lab test results can be obtained through the Life In A Blender Patient Portal system . It is designed to give you 24/7 access to your health information provided by our practice.

Once your account is setup and verified by our office, you can login to view your information under your patient profile. If you want to make any changes, you may submit a message with your request and we will be happy to update your information for you.

You can access the Patient Portal in order to:

  • Access your lab results
  • Communicate with our office staff via the secure messaging feature
  • Request to schedule a new appointment
  • Request a prescription refill
  • Review upcoming scheduled appointments

Your health and your privacy are important to us. The Life In A Blender Family Medicine Patient Portal is powered by Athenahealth, a leading online healthcare administrative technology service provider.

We will need your basic contact information to set up and verify your account. Your information will be kept secure under the HIPAA Privacy Rules.

To login to the Patient Portal, you will need your username and password information to log in. The first time you access the system, you will need to create your account. Click the link that says “Sign up today” to create your unique login username and password. Security questions are designed to protect access to your information, and you will need to answer the security questions to reset your password in the future. If you forget your username please contact us. If you have a username but you forgot your password, select the “Forgot your password?” link to reset your password.

If you have any difficulty logging into the Portal, please:

  • Verify to make sure your username and/or password are entered correctly.
  • Read any error messages that may appear.
  • Check to see if the “Caps Lock” key is activated on your keyboard as all login text fields are case sensitive.
  • Allow a few seconds delay to login after you hit “Enter” or the “Sign In” button in case your internet speed is being slow.
